Note: If the application is filled out and printed on a computer using Microsoft Word, the application format and layout is to be exactly (word for word and design) as the Crime Commission's original application. An electronic submission by fax or e-mail is not accepted.

Effective July 1, 2011 the standard mileage rate will be increased to 55.5 cents from the current rate of 51 cents.
